Please indicate how you heard about Canines with a Cause, and include any questions you may have for us:

UNDERSTANDING THE COMMITMENT: Please read questions from 1 to 13 and answer YES or NO at the bottom of the form.

CWAC requires that:

  1. You take the puppy in public social outings for a minimum of 3 times a week. Socialization is important to the success of our program. Social outings can be restaurants, grocery stores, malls, public events, public transportation, elevators, schools, offices and so on.
  2. The puppy is not left alone for longer than 3 hours during the day.
  3. The puppy is crated at all times when unsupervised. Combination of crate breaks and use of 'umbilical leash' (puppy's leash is attached to puppy raiser's waste thus able to supervise puppy while doing house work) is optimal.
  4. You groom the puppy regularly.
  5. You commit to the CWAC Puppy Raiser Guidelines and specific instructions provided by CWAC regarding the care, training and wellbeing of the CWAC puppy.
  6. The puppy is never left unattended in the backyard, pool, near a lake, even in fenced yards as his/her safety could be at risk and undesirable behaviors can develop I.E. digging, chewing trees and plants etc.
  7. The puppy is never encouraged and allowed to go on mechanical escalators & revolving doors.
  8. You attend weekly (or bi-weekly) organized training sessions.
  9. You use only the equipment and methods of handling and training laid out by the CWAC head trainer.
  10. You give the CWAC puppy only the dog food provided by CWAC.
  11. You complete and provide regularly monthly reports that records the progress of the puppy.
  12. You provide the CWAC puppy with at least 3 good outdoor walking exercises during the day.
  13. You understand that the puppy has a projected turn in date but this could be changed at any point in time due to training needs of a specific puppy, decisions to add the puppy to the possible breeding program or any number of factors deemed necessary by the CWAC head trainer.
space Are you able to commit to that? (1 to 13):space Yes No
